ICA Admission Requirements & Process

Step 1

Submit the following admission requirements:

• Form 137 for Recent High School Graduates (original & photocopy)
• Transcript of Records- Professional & College Level Students (original & photocopy)
• Certificate of good Moral Character
• 2 pcs. recent 2x2 ID picture
• NSO Certificate of Live Birth or Marriage Contract (original & photocopy)
• Laboratory Results
  • Drug Test
  • Hepa A, Hepa B Results (HBsAG, ANTI-HAV lgm)
  • Chest X-ray
  • CBC, Urinalysis, Stool

Step 2

• Submit all requirements to ICA Administrative Office
• Fill up the Application for Admission Form
• Undergo the admission interview with the ICA Director or designated officer
• Submit Laboratory results to the University Clinic
• Secure admission car from ICA office
• Proceed to Business Office for payment

Step 3

• Take the Psychological Test (c/o Guidance Office) Php 300.00
