Audit Program of La Salle Bacolod is offered to those who wish to take refresher in any subjects available in the current semester. This is a non-credit program.
ADMISSION STEPS:
STEP 1: Create your Online Application for Admission through: https://uslscloud.pinnacle.com.ph/lasalle/index.php then click APPLY HERE.
STEP 2: Upload your Online Application for Admission
- Grade 12 Report Card (1st and 2nd Semester) - duly signed by the school principal
- Certificate of Completion
- NSO or PSA Birth Certificate
- Admission Fee Receipt (P 300)
- Signed Undertaking Form - get downloadable forms here
- Medical Clearance from the University Clinic – submit the copy of the following to the clinic for clearance:
- Signed Physical Examination Form (to be accomplished outside USLS) – get downloadable forms here
- Chest X-Ray Result (valid within 6 months prior to Admission Application)
Additional requirements of Students Coming from Schools Abroad:
- Grade Interpretation or Equivalence
- Course Syllabus or Subject Description
- Recommendation Form in Narratives
- Foreign Passport and/or Philippine Passport
- Medical Insurance
- Certificate of Dual Citizen- for those declaring that they have dual citizenship
