Please take note that that the Admission and Scholarship Administrative Office (ASAO) will implement the Appointment System starting April 1, 2021 onwards. Submit the following scanned copy of requirements through email at heu.admission@usls.edu.ph for assessment/ evaluation. You will get notification within five (5) working days.

Admission and Enrollment starts in JUNE 2021


Step 1 : FILL OUT ONLINE APPLICATION FORM

Step 2 : SUBMISSION OF SCANNED ADMISSION REQUIREMENTS TO
COLLEGE ADMISSION OFFICE EMAIL ADDRESS (heu.admission@usls.edu.ph)

SUBJECT: APPLICATION FOR SUPPLEMENTAL
NAME OF APPLICANT:

  • Original Transcript of Records

  • Official Transfer Credential (applicable non-La Sallian graduates only)

  • Online Application Form (Verification and Data Consent)

  • Undertaking Form with 1x1 ID photo

  • 1 Accomplished Recommendation form 

  • PSA Birth Certificate (1 original and 1 photocopy)

  • Admission Fee Receipt (P 200)

  • Physical Examination Form (accomplished by any physician outside University) and Chest X-Ray Result

    • For Nursing Applicant’s additional requirements: HBsAg Result

    • For Dual Citizen and International students’ additional requirements: Photocopy of Foreign and local passport bio-page, Certificate of completion/ Diploma, Course Syllabus with grade interpretation (all documents must be in/ with English Translation)


Step 3 : ISSUANCE OF ADMISSION CARD 

Admission card will be released within five (5) working days upon submission of completed admission requirements through email.


Step 4 : ENROLLMENT

Kindly pay Upon Enrollment at USLS Cashier or recognized payment centers.

 

Step 5 : SUBMISSION OF SCANNED/ CLEAR COPY OF ADMISSION CARD AND
OFFICIAL UPON  ENROLLMENT RECEIPT

Step 6 : ACKNOWLEDGEMENT

You will receive acknowledgement letter and confirmation of enrollment with class/ block schedule.
