ADMISSIONS STEPS:


STEP 1: Submit a clear copy of your Transcript of Records and Honorable Dismissal for initial evaluation to heu.admission@usls.edu.ph (Subject: Transferee - State Your Course) 

STEP 2: Pay at USLS Cashier for Nursing Aptitude Test (NAT) 300 pesos. And get a schedule for Nursing Aptitude Test (NAT) at the Guidance Evaluation Center Bring 2pcs of 2x2 picture and NAT Receipt.

STEP 3: Once you receive a confirmation to proceed for admission on email, create AIMS Account thru: https://aims.usls.edu.ph/lasalle/applicants/index.php
*Take note of your Student Application Number (SAN) sent to your registered email.

STEP 4: Pay Admission Processing Fee (P300) & Entrance Exam Fee (P210) by using your SAN. Take an Entrance Test.

Option 2: Visit our Guidance and Evaluation Center for instructions.

*Note: Students who qualify for the NAT cut-off score can take the Entrance Exam only. All fees are non-refundable.

STEP 5: Fill out information and upload requirements to your AIMS account:

  • Transcript of Records
  • Transfer Credential or Honorable Dismissal
  • NSO/ PSA Birth Certificate or Foreign Birth
  • Signed Undertaking Form
  • Admission Fee Receipt
  • Entrance Test Receipt and Result
  • Medical Clearance from USLS Clinic 
    • Chest X-Ray
    • Accomplished Physical Exam Result
  • Signed Recommendation Form (sealed in a long envelope) 
  • For Nursing Applicants only - Additional Requirements: 
    • NAT Test Result 
    • Vaccination Card (1st and 2nd doses)
    • HBsAg Result
    • PhilHealth Card or MDR

 

Additional Requirements for Students coming from Abroad: 

  • Grade Interpretation or Equivalence 
  • Course Syllabus or Subject Description
  • Recommendation Form in Narratives
  • Foreign Passport and/or Philippine Passport
  • Medical Insurance
  • Certificate of Dual Citizen -for those declaring having dual citizenship status
     

STEP 5: Release of Admission Card

Upon completion of your complete admission requirements, you will receive your admission card within 3-5 working days on your email or you may get it from the Admission Office. Please proceed to ENROLLMENT right away. For enrollment instructions, please visit https://www.facebook.com/usls/registrar.
 

*Kindly submit all admission requirements in hard copies to the Admissions and Scholarships Administration Office (ASAO) on or before the start of classes, as this will serve as your clearance to enroll in the second semester AY 2024-2025.

ENROLLMENT PROCEDURES AND SCHEDULE

Enrollment for Incoming Freshmen begins June 10, 2024. For freshmen enrollment steps, check this link: https://sites.google.com/usls.edu.ph/heu-enroll-guide.

Enrollment for other year levels will start on July 29, 2024. Enrollment steps for continuing students will be released soon by the University Registrar.

Classes begin on August 5, 2024.
